INFORMATION FOR SPONSORS:

Events may be sponsored by clubs or by individuals. All forms should be sent to the Sanction Coordinator

(see address below). Please use current forms. These are marked with "1/2009" in the lower right corner, and

can be obtained from the Sanction Coordinator. (If downloaded or emailed forms need reformatting after being received, the margins are 0.5 at top, 0.25 at bottom, and 0.75 on the sides.) Please discard any earlier forms.

Trial Sanction Form

The completed sanction forms are returned to the CFTC Sanction Coordinator at least 60 days prior to the event, along with the sanction fee of $20.00 for a trial. A two or more day event held over consecutive days is included in one sanction. All sanctioned events will appear in the events calendar on the CFTC website and in the newsletter. A copy of the trial entry form should be sent to the CFTC at least two weeks prior to the event. NOTE: VINTAGE/PASTORAL sanction forms must include: (1) a step-by-step description of the course, (2) a diagram of the course, and (3) a score sheet set up in the format of the VINTAGE score sheet. PASTORAL forms must include what type and location of sort will be used.

Trial Score sheets

Master forms are provided, from which the required number for the event can be made. There should be an original and one copy for each participant. (NCR forms can be prepared at many copy stores, or carbons may be used, or photocopies made after the event.) After the event, the completed original is given to the owner and a copy is kept by the sponsor for at least one year (in lieu of keeping copies of the score sheets, the sponsor may instead keep either copies of the Trial Recording Forms or a completed catalog). The score sheets are not sent to CFTC. Where applicable, the name of the sponsor can be filled in before the copies are made.

Trial Recording Forms

Master forms are provided, from which the required number of copies can be made (this is an 8-1/2 x 11 page with two forms to the page). Again, where applicable, the name of the sponsor can be filled in before the copies are made. These are the forms that are actually sent to the CFTC Sanction Coordinator for use in recording titles. Only one set need be sent. The Recording Forms can be filled out by the sponsor from entry information, or can be included as part of the entry package for the entrant to fill out. In the latter case, the entrant fills out the top part of the form (the information regarding the dog, address of owner, etc.); the entrant then gives the partially-completed form to the event secretary, so that the bottom part can be filled out with the results at the conclusion of the trial or test. A separate form must be completed for each run. Once completed, the form is sent to the CFTC Sanction Coordinator for recording, along with the recording fees of $3.00 per qualifying run. Only the forms for qualifying dogs need be sent in, although the report forms (see no. 4 below) must include results for all dogs, whether qualifying or not. (Note: The recording forms are two to a page – the page will eventually be divided so please be sure to fill out bottom form fully even if information is duplicated from the top form – notations such as "see above" will not work because the page at some point will be cut in half and the two forms separated.) The Recording Forms must include an CFTC tracking number. Forms for dogs lacking tracking numbers should be sent in with the other forms, but there may be a delay in recording those forms while the number is being checked or issued.

Trial Report

These forms provide an overview of the event. The original is sent to the CFTC Sanction Coordinator after the event, along with the Trial Recording Forms and the recording fees. (Note: as specified in the rules, a fine will be assessed for late submission of forms and fees). As indicated on the Report Form, in lieu of listing the results on the form itself (that is, a list of the dogs taking part in the event and the results for each), a completed catalog can be substituted if available. A catalog may not, however, be substituted for the Recording Forms.

In summary, the CFTC Sanction Coordinator is to receive: (1) The Sanction Form and fee 60 days prior to

the event; (2) a copy of the trial entry form at least two weeks before the event; and, within 15 days after the event (3) completed Recording Forms for all qualifying dogs, with fees; and (4) the completed Report Forms.
